ELDER ABUSE 101
It is important that all of you know what truly happened; not the defamatory fable that Michael Flaherty continues to tell. In what began as an attempt to find fault in me, actually exposed their own faults and the true Evil that lurks within the Taylor Community administration. As you look through the Lifeline records here, ask yourself: How long is it acceptable for your Lifeline to be disabled? Quoting former Taylor Community Trustee, Chris Volpe, "There should not be a moment when your Lifeline is not working". Yet, he nor any of the Trustees have done anything to upgrade the Lifeline systems or address Taylor Community staff's attempt to kill me. Despite the mountains of irrefutable digital evidence we have compiled, no apologies have been issued to me or my family. Just continued lies and denials that there are any issues with their system and refusal to upgrade their antiquated, unserviceable equipment.
We were all led to believe that our Lifelines were to be fully functional 24/7/365 days a year.
On the morning of April 17th, 2018, I found that not be the case. When I awoke that fateful morning, I was feeling very ill and was unable to stand up out of bed. After repeatedly pressing my Lifeline pendant without a response, I slid from my bed to the floor and began the long crawl to my front door. In what seemed like hours, while going in and out of consciousness, my own sheer will to live gave me enough strength to get me to my front door. I was found laying halfway out in the hallway by my neighbor, who called 911, and I was then hospitalized with pneumonia. It wasn't until March 29, 2019 when I received my 2018 records from Lifeline Corporate that I realized how truly fortunate I was to even be alive. Hank Offinger and Heather Bell‘s intentional negligence in maintaining my Lifeline, likely would have caused my death had I not had the sheer will to get to my front door. My Lifeline records show that my equipment was malfunctioning and/or completely disconnected for the majority of 2018! Hank and Heather ignored the Failed Supervision com-log alerts in the weeks leading up to this incident and for over two months afterwards. The records shown in the gallery below are just a sample of the recklessness exhibited by Hank Offinger and Heather Bell all under the watch of CEO Michael Flaherty. The Trustees and Board of Directors condoned their actions and Flaherty has repeatedly stated his support for the staff’s performance even after these records were exposed. None of their actions bear any semblance to their mission statement; as the records show, they provided zero support for my independence, health, or dignity.
I can’t emphasize it enough, for all residents to get your Lifeline records. Michael Flaherty and Hank will tell you there is nothing to worry about... don’t believe them. If your base unit is a 6700 model, insist on getting it replaced. Under their contract with Lifeline, Taylor Community is required to keep the most updated equipment available to you. Check your Lifeline pendant, on the back will have a replacement date. If it is past that date, insist on getting it replaced regardless of what they tell you. Your life depends on it. For those of you that use a walker, it is most important that you get a wrist fall detection bracelet immediately. People have been choked to death from strangulation by the necklace pendant (See simulation in Gallery below) Regardless of what anyone says, the breakaway isn’t always reliable. It’s your life and your right, as well as Taylor's responsibility to provide you with the latest technology available. The majority of Lifeline Units at Taylor are 3-4 generations old and are not serviceable by Lifeline anymore. Don't let Hank, Heather, or Flaherty lie to you like they tried with my family, my doctor, and my therapists.
Here are some of the Codes that almost became my death sentence, most Woodside residents' Com-log Reports from 2018 will have one or more of these, but you’ll only know if you demand to see them. If you have any issues getting them, call my son Mike at 210-835-7602.
He has direct contact with the Lifeline Representative that got my records for me.
FAILED SUPERVISION: Pendant is not communicating with Base Unit, if pressed no signal will be sent and no help will arrive. You will die.
NO A/C HOME UNIT, LO BATT-UN: Base Unit is disconnected from wall socket and battery running low, will die in 36 hours if not plugged back in. Once battery dies, no signal can be sent and you are without the Lifeline service. As the battery dies so will you.
ISSUE NOT RESOLVED: Hank/Heather are ignoring the warning signals that there is a problem with your equipment and have left it to remain malfunctioning. They want you to have a catastrophic event.
CONDEMNATION LETTER
After securing the fraudulently obtained Doctor’s recommendation, Taylor Community Care Management Director, Hank Offinger, emailed a letter to my sons attempting to garner their support to put me into Assisted Living.

This failed miserably, as you shall see by the email chain provided in the gallery below. His claim about my inability to take my meds was only one of the many glaring untruths that stood out while reading it. The fact is, my Sons were visiting me every other month and knew my condition was actually improving.
Here is the truth about these false allegations:
🚩When questioned about specific doses I had supposedly missed, there was nothing to substantiate the claim. Walmart Pharmacy has a system that monitors my meds, they found that I was at 95% compliance, completely refuting the percentages that Hank contradicted himself on a number of times. (First it was that I was supposedly missing 71% of them; then I was only in compliance 75%) It's funny how things get confusing when all you do is lie. Since Taylor Care Nurse, Heather Bell, was hired to manage ALL my medications, per my Doctor’s request, requiring her to reorder and pick up as needed; if there were any failures to do so, it would have been her failure in doing her job.
This included, but was not limited to:
- Symbicort
- Combivent
- Montelucast
- Losartan
- Potassium and all my Supplements
Even more troubling is the fact that she knowingly had me soley on Combivent (a very potent emergency inhaler) and overdosed me for over 8 months while she repeatedly failed to supply me with Symbicort (my normal daily inhaler). Not only was this irresponsible but potentially deadly.
🚩Hank’s allegation that I will “eat only yogurt” is ludicrous. The fact is, the probiotics in the yogurt were actually part of my treatment for a reoccurring C-Diff infection that I suffered through. Hank has no clue what I was eating on a daily basis, and when confronted, had nothing to back up the allegation. Food was left in the fridge by my sons after the Holiday and was way more than one person could eat in a week. I didn’t forget it was there, my sons who live out of state called Heather to do a courtesy check and throw out the excess. Once again Heather and Hank seized the opportunity to turn it into something negative against me. This is what you can expect if you hire “Taylor Care”.
🚩The allegation of having a number of falls in the past six months was another blatant outright LIE. No documented incident or injury report on record. Furthermore, if Hank was so concerned about me being a fall risk, why wasn’t I given a Fall Detection bracelet instead of the recalled, malfunctioning pendant (choking hazard) that I wore around my neck? His actions don't exhibit any concern for my safety.
🚩Heather Bell’s attempt at cognitive testing was contraindicated.....without accommodation for my hearing impairment, my age, the effects of the heavy medications I was on during my C-Diff treatment, as well as the stressful situation I had just experienced. According to MOCA headquarters, the test is only a tool and not anywhere near a concrete assessment of my cognitition. Apparently, Hank and Heather didn't get the memo. (my most recent MMSE test score was 28/30 taken by a licensed Neurologist, showing above normal cognition) While Heather claims to be certified to administer a test; she is not licensed to interpret any cognitive test. The Real Question is: If they truly believed that I was as “impaired“ as they alleged, why did they allow me to drive on their campus on a daily basis? Or was that part of the plan to get rid of me?
Everyone residing at Taylor Community is potentially going to be on the list for Assisted Living whether you signed up for it or not. So, if and when your children receive a letter like this or begin getting pressured, you'll know it's time to “Lawyer-up“. Given the information you are being presented, it might impel you to get ahead of this now. Don't waste your time going to the Trustees, Board of Directors, or any State Agencies, they are not going to step in. It seems that through the hefty "donations" to these entities, Taylor Administration has been able to escape prosecution for their unethical and criminal acts.
Having all the information he needed and with the regulatory duty to intervene, the NH State Insurance Commissioner‘s refusal to act is appalling. The omission of damning factual evidence that destroyed Taylor’s credibility and allegations, and with Susan Denopoulos going unmentioned and eliminated from Investigation, indicates a coverup that threatens the safety of every resident in this facility.

The complaint to the Insurance Commission was based on facts , not mere allegations. We submitted Hard Factual Documentation and Technical Data that fully substantiate our claims. None of which was mentioned in their Fraudulent Report. The "investigators" merely relied on conjecture from their interviews with Taylor Staff and considered them as facts. While every element of our complaint was supported by documented factual evidence which refuted all their claims.

No mention of Susan Denopoulos, Head of Clinical Operations and the main suspect in the crimes who, along with Michael Flaherty orchestrated the multiple events that almost killed me. From unplugging my Lifeline medical alert system, to intentionally overdosing my medications and submitting false information to my Doctor. Her involvement was at the top of the list, yet they let her conveniently “retire” 2 weeks into the investigation ie; Elder Abuse Cover Up operation.
Does it strike anyone as curious why it took 52 days after my Son and I were interviewed, to interview Hank and Heather? And 54 days before they interviewed Flaherty and Soucy? That gave them exactly 2 business days to make a decision and tell me whether I could remain in my apartment or not. The timeline speaks for itself; they had no intention of finding in my favor from the very beginning. Never interviewed or mentioned Susan Denopoulos, my son Gerry, my Doctor, my Physical Therapist, nor any representative at Lifeline. All of whom had important factual & enough damming information to prove Elder Abuse and put these criminals in prison.

As evidenced by the text, Hank and Heather continued to create a false narrative. Fact is, Hank never attempted to intervene when Genesis staff was neglecting and refusing to shower me, along with isolating me in my room attempting to de-condition me. Though he wasn't an employee of Genesis, by contract he was my representative from Taylor, who should have had my best interest at heart. When it came down to my discharge meeting from Genesis, Hank made sure to be in attendance and was visibly disappointed when the therapists both agreed that I should stay living independently and not be subjected to Assisted Living.

As previously stated the confusion was with Heather on how to properly monitor medications. Her selective recall of the facts is egregious and her attempt defer blame is irresponsible at best. Needless to say, her haphazard approach in doing her job could have killed me. Apparently I put too much trust in someone who I believed to be a medical professional qualified to monitor my medications.
Hank also had a habit of backpedaling whenever confronted and stood true to that role in his interview. On 12/14/18 he clearly stated in the email to my sons, that Taylor Care was providing the services that I needed to remain living independently in my apartment at 227 Woodside.
There is No Confusion in what he wrote. Yet the Insurance Commission allowed him and everyone interviewed to twist their story to fit the narrative of the Cover Up.

If this was a formal deposition, Flaherty would be in jail for the lies he told here and the actions by his Staff that he self admittedly supported.
The TRUTH is: My Son Mike requested that no one was to discuss moving me to assisted living unless Mike was present as my POA. Hank disregarded his request and entered my apartment unannounced and attempted to persuade me to go into Assisted Living.
Ed Soucy who? He had nothing to do with the investigation, unless it was for administering the payoff.
Fact: My assertion that they needed me out of my apartment is confirmed by the information Soucy gave about of the waiting list for rental units at Woodside.
Fact: I received the termination notice on April 2, 2019, two days after I gained possession of the records proving Taylor’s Lifeline negligence during the crawl for my life incident that occurred in 2018. All of which is documented in the Timeline provided to the State Insurance Commission. This was pure retaliation by Flaherty in a hasty attempt to mitigate damages. The Insurance Commission failed to acknowledge the facts and failed miserably in doing an honest investigation. There was never a legal eviction done, just threats to shut off my Lifeline and all other services. They forced my departure by threatening my safety.

The only reason to keep this confidential, would be to Cover Up the corruption in the NH State Insurance Commission.
I was never given an explanation as to why the “investigators“:
1) Omitted the Main Suspect, Susan Denopoulos.
Possibly because her emails alone destroy Taylor’s credibility.
2) Disregarded the timeline that showed the retaliation by Flaherty in terminating my contract 2 days after I secured the damning Lifeline records, proving Taylor Community's negligence.
3) Disregarded Hard Factual evidence we presented. Taylor’s documents actually contradicted much of their own statements.
4) Refused to reopen the case when questioned about the blatant Omission of Evidence.
The NH State Insurance Commissioner John Elias was contacted and informed of his department’s glaring Errors and Omissions in the investigation, but he too refused to reopen. Either they are inept at conducting a thorough investigation or they are being paid off by Michael Flaherty and the Trustees.
UPDATE: The exposure of his criminal involvement in falsifying an investigation has successfully removed John Elias from his position. It's just a matter of time for the Trustees to remove Flaherty.

Michael Flaherty stated, he “knew all about my situation and fully supported the work of Hank Offinger and Heather Bell“. Therefore I can only conclude that:
*He knew my Lifeline was unplugged every January for the past 3 years.
*He knew that Hank Offinger and Heather Bell intentionally ignored the comlog warnings about the disconnections and pendant malfunctioning for months at a time.
*He knows that I am fortunate to be alive due to his Staff’s negligence.
*He knew about the Medical/Healthcare Records fraud committed by Heather Bell when she submitted false information to my Doctor for a recommendation for Assisted Living.

Susan stated that my Therapists and Primary Care physician came to her recommending me for Assisted Living.
And... it was conclusive that I needed 24/7 care.
After my Sons requested the conclusive evidence to support her claims, she had nothing but fabricated nurse notes with no substantiation.
The documented official phone note between Heather Bell and my doctor‘s office confirms that they in fact went to my Doctor requesting the AL recommendation, using false information to influence his decision.
After reporting Taylor’s illegal eviction, Medical Records Fraud, and Reckless Endangerment to the State Insurance Commission, Denopoulos "retired" and her existence was intentionally omitted from their investigation. An obvious attempt to cover up Taylor Community Staff's Negligence.
HANK OFFINGER, Director of Care Management
Hank Offinger began his campaign of putting my husband Walt and I into Assisted Living back around 2014. Walt did not appreciate Hank‘s harassment and made it clear that he would leave before he ever let them lock him up.
After Walt’s death, I moved to Woodside where Hank was able to prey on me more easily. There were occasions where he did assist me, though it has become clear to be an attempt to earn my trust in order to infiltrate my life and create a false narrative.
Hank is also the head of Programs for the Lifeline Emergency Alert System. (See the Lifeline Slideshow below) The records speak for themselves about how he Failed at his job in that department. CEO Michael Flaherty supports him exclaiming a “proficiency in his role“. Is it his role to Recklessly Endanger the Senior Citizens of this retirement community? According to the Director of Programs at Lifeline Corporate, Shawn Mickleboro, the system in the entire Woodside building was not properly connected for much of 2018 and into the first three months of 2019. It would still be in disarray to this day, had it not been for my 2 sons reporting the disconnection of my unit.
The records show Hank’s recklessness and a complete disregard for my safety. I would recommend for everyone to get your Lifeline Records immediately and see what is going on with your unit. Do not take Hank, Heather, or MF’s word for it. You have a right to those records. Above all do not trust them if they tell you they can only get 90 days back. I got 7 years back, so should you.
If your base unit is a 6700 model, it is 15 years old technology, demand for it to be replaced with the 6900. Your life depends on it!
HEATHER BELL, Care Management Nurse
Heather Bell became the new Nurse at Woodside almost overnight when Nurse Jacinda Lemay left due to “pressure to do things that would compromise her Nursing License”
My experiences at Taylor fully corroborate that statement. After my hospitalization and subsequent C-Diff infection which was only getting worse at Genesis Rehab Center, I moved back to my apartment at Woodside. Being on a regimen of 4 times a day antibiotic along with my other meds, my sons and Doctor agreed that I should have Heather monitor my meds as backup. Little did I know this was going to be the beginning of the end of my independence at Woodside. After just the first week, Heather was already requesting a recommendation from my doctor for my movement to Assisted Living, but was denied. There is no question in my mind that Taylor Care is a bait and switch operation. The sales pitch is that you can pick and chose from their menu of services. But that seems to be just the hook. Once I let them into my life, it was only a matter of time before the sentencing letter was penned. In my case it was 6 months and Heather had made up enough lies to feed my Doctor and obtain a recommendation. Your results may vary.
It must seem so routine to them, that they didn’t even cover their trail. After all why should they, since they are protected by the NH State Insurance Commission and every State Agency all the way up to the Governor.
In summary...Heather Bell:
*Submitted false medical information to my Doctor for financial gain, ie; Medical Records Fraud.
*She and Hank knowingly ignored comlog Warnings leaving me in a situation that could result in serious injury or death, ie; Reckless Endangerment
*She overdosed and mis-administered my medications. ie; Malpractice/Negligence
Taylor Community CEO, Michael Flaherty claims that Heather is “proficient in her role.” Then it must be her role to violate every Code of Ethics of the Nursing Profession. Having been under her "care", I feel fortunate to have survived. After reporting her, it will be abhorrent if she is allowed to keep her license.
RISK MANAGEMENT NIGHTMARE
Located nestled in the hillside of Laconia with the total absence of a security gate or security officers, you might wonder who is protecting these 400 defenseless Senior Citizens? If you were to inquire, Taylor claims that the facilities staff are there watching over the facility. (landscapers and maintenance crew) I find it hard to believe that the 6-10 guys I’ve seen blowing leaves or snow around on a daily basis have any sense of what’s going on. Nor do the guys doing maintenance inside the units, have a clue what’s going on outside.
Of course they say there’s always your Lifeline that you can depend on as your fail-safe in case of emergency 24/7/365. That may be a valid point if and/or when it’s working. Read on......
Had I not found the strength of will to crawl for my life when I fell ill last year, I would not be writing this today. You know the facts by now.....Up until the morning of April 17, 2018, I believed that my Lifeline pendant was my fail safe when I needed it. As it turns out, Hank Offinger and Heather Bell were intentionally ignoring the daily Failed Supervision Comlog warnings, putting me in a situation where I could have died in my room. The records show that they continued to ignore the Comlogs well after I was hospitalized and returned to my Apartment.
After attaining my Lifeline records from 7 years back, the documentation showed a pattern of behavior that my machine was disconnected in the month of January for three years in a row from 2017-2019, and malfunctions went unresolved for over to two full months at a time. Taylor’s negligence is further exemplified when Lifeline Corporate informed them on two other occasions that my antiquated unit needed to be replaced immediately. Hank and Heather refused to comply. So much for caring about my safety.
Whoever is insuring this facility needs to know about the lack of security as well as the lack of maintenance on your Medical Emergency Alert system.
